Case Management

Assessment, authorization, and monitoring of a variety of in-home supportive services. Operates under contract with the Illinois Department on Aging and the Suburban Area Agency on Aging. Provides assessment and linkages to home delivered meals, housekeepers, homemakers, adult day care, respite care, senior companions, senior advocacy, money management, family caregiver program, Medicare fraud, abuse and education.

Information and Referral - Provides information and referral on programs and services for elderly who live in the South suburbs. This includes the townships of Bremen, Bloom, Calumet, Thornton and Rich.

Case Management - Assessment, authorization, and monitoring of a variety of in-home supportive services. Operates under contract with the Illinois Department on Aging and the Suburban Area Agency on Aging. Provides assessments and linkages to home delivered meals, homemakers, adult day care, transportation, senior companions, call reassurance, money management, Medicare/Medicaid fraud and senior advocacy. This unit also provides service coordinators to low-income housing complexes.
