Family Case Management and Better Birth Outcomes

Our team provides free case management to income qualifying pregnant women and children under the age of one. Support includes home visiting, referrals to health care, assistance obtaining public, and prenatal and parenting classes.

Our Better Birth Outcomes program provides free case management to income qualifying pregnant women deemed “High Risk”. Services are provided to women residing on the far south side of Chicago.
