Description: 1. Seeks out potential Supplemental Nutrition Assistance Program (SNAP) clients and their families through face-to-face interactions, presentations and fairs;
2. Completes assessment and reviews client documentation to determine eligibility for SNAP and other benefits;
3. Participates in community outreach efforts and attends community events and meetings to develop and maintain positive and collaborative relationships with community partners to provide services for current and prospective clients;
4. Schedules appointments with clients at assigned locations and/or outreach locations and immediately informs supervisor of any incidents or crisis situations;
5. Works collaboratively with internal and external partners to schedule events, programs and activities;
6. Conducts short-term case management to address clients' needs and tracks the application process, informs participants of benefit renewal dates and follows up on any issues;
7. Serves as the first point of contact for clients and case workers and responds to all inquiries, calls and/or emails within 24 hours of receipt;
8. Maintains the confidentiality of client information in compliance with Agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies in the course of performing job expectations;
9. Maintains up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies in the course of performing job expectations;
10. Must maintain a flexible work schedule to work evening hours or special events, as needed.
